Abstract

Arterial hypertension is one of the most prevalent chronic cardiovascular disorders and represents a major global public health challenge because of its substantial contribution to cardiovascular morbidity, cerebrovascular complications, renal impairment, and premature mortality. The disease is characterized by a persistent elevation of arterial blood pressure resulting from the complex interaction of genetic predisposition, environmental influences, behavioral characteristics, metabolic abnormalities, and neurohumoral regulatory mechanisms. Although hypertension may remain clinically silent for a prolonged period, sustained elevation of blood pressure progressively affects target organs, including the heart, brain, kidneys, retina, and vascular system. Consequently, early identification of individuals at increased cardiovascular risk and implementation of effective preventive measures are essential components of modern healthcare. The etiology of arterial hypertension is multifactorial. In the majority of patients, hypertension develops without a single identifiable cause and is classified as primary or essential hypertension. Its development may be influenced by hereditary susceptibility, excessive dietary sodium intake, insufficient physical activity, unfavorable dietary patterns, excessive body weight, chronic psychological stress, inadequate sleep, tobacco exposure, and metabolic disturbances. Secondary hypertension may arise in association with renal diseases, endocrine disorders, vascular abnormalities, certain medications, and other identifiable clinical conditions. Understanding these etiological factors is particularly important because many of them are potentially modifiable through lifestyle interventions and appropriate medical management. The pathogenesis of arterial hypertension involves disturbances in vascular tone, renal sodium and fluid regulation, sympathetic nervous system activity, the renin–angiotensin–aldosterone system, endothelial function, and vascular remodeling. Persistent activation of these mechanisms may increase peripheral vascular resistance, promote sodium retention, alter vascular elasticity, and maintain chronically elevated blood pressure. Prolonged hemodynamic stress subsequently contributes to structural and functional changes in target organs, increasing the probability of myocardial hypertrophy, ischemic cardiovascular events, heart failure, stroke, chronic kidney disease, and other complications. Modern prevention of arterial hypertension requires an integrated and individualized approach. Primary prevention should focus on maintaining a healthy body weight, reducing excessive sodium consumption, following a balanced dietary pattern, engaging in regular physical activity, avoiding tobacco exposure, limiting harmful lifestyle factors, maintaining adequate sleep, and managing psychological stress. Regular blood pressure measurement and cardiovascular risk assessment facilitate early identification of individuals who may benefit from preventive interventions. For patients with established hypertension, appropriate antihypertensive therapy combined with sustained lifestyle modification is essential for reducing cardiovascular risk and preventing target-organ damage. The purpose of this article is to provide a comprehensive overview of the principal etiological factors and pathogenetic mechanisms underlying arterial hypertension and to examine contemporary approaches to its prevention. Particular emphasis is placed on modifiable risk factors, early detection, lifestyle-based interventions, cardiovascular risk reduction, and the importance of integrating preventive strategies into routine medical practice. A comprehensive prevention-oriented approach may contribute to improved blood pressure control, reduction of hypertension-related complications, and preservation of long-term cardiovascular and renal health
